目的分析320排CT灌注成像在肺结节的良恶性鉴别诊断中的应用价值。方法选取40例我院接受治疗的肺结节患者,并于治疗前进行320排CT灌注扫描,得到肺动脉灌注值(PF)、支气管动脉灌注值(BF)、灌注指数(PI),分析灌注参数差异,对其所得的诊断结果与患者病理诊断进行比较。结果 320排CT灌注成像在肺结节的良恶性鉴别诊断中恶性结节BF值明显高于良性结节,PF值及PI值良性结节高于恶性结节。良恶性的鉴别诊断敏感性为94.59%(35/37),准确性为90%(36/40),阳性预测值为94.59%(35/37),与病例诊断的金标准100%比较,P>0.05,差异无统计学意义。结论使用320排CT灌注成像在肺结节的良恶性鉴别诊断中,具有很高的诊断率及临床应用价值,值得推广。
Objective To analyze the value of 320-slice CT perfusion imaging in the differential diagnosis of benign and malignant pulmonary nodules. Methods Forty patients with pulmonary nodules who were treated in our hospital were selected and scanned with 320-slice CT perfusion before treatment. Pulmonary arterial perfusion (PF), bronchial arterial perfusion (BF) and perfusion index (PI) were obtained. The perfusion parameters Differences, the results of its diagnosis and pathological diagnosis of patients were compared. Results 320-slice CT perfusion imaging in benign and malignant pulmonary nodules in the differential diagnosis of malignant nodules BF value was significantly higher than benign nodules, PF value and PI value of benign nodules than malignant nodules. The sensitivity of benign and malignant differential diagnosis was 94.59% (35/37), the accuracy was 90% (36/40), the positive predictive value was 94.59% (35/37). Compared with the gold standard of case diagnosis, P > 0.05, the difference was not statistically significant. Conclusion The use of 320-slice CT perfusion imaging in the differential diagnosis of benign and malignant pulmonary nodules, with high diagnostic rate and clinical value, is worth promoting.
