目的:观察加味苍附导痰汤联合炔雌醇环丙孕酮片治疗多囊卵巢综合征(PCOS)的临床疗效。方法:选择我院门诊就诊的PCOS 100例,随机分为观察组与对照组各50例。对照组于月经周期内或黄体酮撤退性出血第5天口服炔雌醇环丙孕酮片;观察组在口服炔雌醇环丙孕酮片的基础上,加用加味苍附导痰汤治疗。两组均21天为1个疗程,连续3个疗程。比较两组临床疗效,分别于治疗前、疗程结束时及1个月后随访时测定黄体生成激素(LH)、尿促卵泡素(FSH)、雌二醇(E2)、催乳素(PRL)及睾酮(T)水平;采用Ferriman-Gallwey评分标准评估多毛体征,痤疮评分系统记录痤疮严重程度,中医症候评分表评估月经不调发生情况以及卵巢体积变化情况。结果:观察组总有效率88.0%,显著高于对照组的60.0%(P<0.05)。两组疗程结束后及1个月随访时血清LH、FSH、E2、PRL、T水平均较治疗前显著降低,卵巢体积较治疗前显著缩小,毛发评分、痤疮评分、月经不调评分均较治疗前显著降低(P<0.05)。疗程结束时,观察组上述血清指标及卵巢体积均显著低于对照组(P<0.05);1个月后随访时,观察组血清LH、E2、PRL、T水平及毛发评分显著低于对照组(P<0.05)。结论:加味苍附导痰汤联合炔雌醇环丙孕酮片治疗PCOS疗效优于单用炔雌醇环丙孕酮片。
Objective: To observe the clinical effects of Modified Couture Dantan Decoction combined with ethinylestradiol and cyproterone tablets in the treatment of polycystic ovary syndrome (PCOS). Methods: 100 cases of PCOS in our outpatient clinic were randomly divided into observation group and control group with 50 cases each. The control group oral ethinyl estradiol cyproterone tablets in the menstrual cycle or progesterone withdrawal bleeding day 5; observation group oral ethinylestradiol cyproterone tablets based on the addition of flavored Cangshen guided sputum soup . Both groups were 21 days for a course of treatment, three consecutive courses. The clinical efficacy of the two groups were compared. The levels of luteinizing hormone (LH), urinary follicle stimulating hormone (FSH), estradiol (E2), prolactin (PRL) and testosterone were measured at the end of the treatment and before the end of the treatment. The TCM symptom score scale was used to assess the incidence of irregular menstruation and the change of ovarian volume. Serum levels of LH, FSH, E2, PRL and T were significantly lower than those before treatment after treatment and at 1-month follow-up. CONCLUSION: Modified Cailian Dantan Decoction combined with ethinylestradiol and cyproterone tablets is superior to ethinyl estradiol cyproterone tablets in the treatment of PCOS.
