[目的]研究大脑实质内孤立性浆细胞瘤。[方法]采用光镜、组织化学和免疫组化染色对2例大脑实质内孤立性浆细胞瘤进行观察,并文献复习。[结果]瘤细胞较单一,为不成熟及接近成熟的浆细胞,呈瘤性增生。甲基-哌络宁(MGP)染色,瘤细胞胞浆呈鲜红色。血管壁及瘤组织间有对刚果红染色阳性的淀粉样物质。瘤细胞示单克隆性轻链kappa阳性。[结论]大脑实质内孤立性浆细胞瘤是一种十分罕见肿瘤,诊断时应排除多发性骨髓瘤侵犯脑实质。此瘤诊断依靠光镜、组织化学和免疫组化染色、临床实验室资料,才能作出确切诊断。
[Objective] To study solitary plasmacytoma in the parenchyma of the brain. [Methods] Two cases of solitary plasmacytoma in the parenchyma of the brain were observed by light microscopy, histochemistry and immunohistochemical staining, and reviewed in the literature. [Results] The tumor cells were relatively simple and immature and nearly mature plasma cells showed neoplastic hyperplasia. Methyl-Peraldline (MGP) staining showed bright red cytoplasm in tumor cells. There was amyloid staining positive for Congo red between the vessel wall and the tumor tissue. The tumor cells show a monoclonal light chain kappa positive. [Conclusion] Solitary plasmacytoma in the parenchyma of the brain is a very rare tumor, and multiple myeloma should be excluded from the invasion of brain parenchyma at the time of diagnosis. The diagnosis of this tumor depends on light microscopy, histochemical and immunohistochemical staining, and clinical laboratory data to make a definitive diagnosis.
