目的通过观察原发性高血压患者臂踝指数(ABI)与颈动脉内膜中层厚度(IMT)及炎性介质的相关性,进一步探讨其在此类患者发病机制中的作用。方法测定48例初诊原发性高血压患者和50例健康体检者的ABI、IMT、超敏C反应蛋白(Hs-CRP)、白介素-6(IL-6)和血压等指标。结果高血压患者的颈动脉IMT、Hs-CRP、IL-6、收缩压和脉压均显著高于健康者(p<0.05);ABI显著低于健康者(p<0.05)。ABI与颈动脉IMT(r=0.401)、Hs-CRP(r=0.549)、IL-6(r=0.467)、收缩压(r=0.345)和脉压(r=0.354)呈显著负相关。结论臂踝指数(ABI)与颈动脉内膜中层厚度(IMT)及炎性介质均对原发性高血压患者外周动脉硬化程度的评价有重要意义,其中臂踝指数无创、简便,适合在高血压人群中作为常规筛查手段来推广。
Objective To explore the role of ABI in the pathogenesis of these patients by observing the correlation between ABI and carotid artery intima-media thickness (IMT) and inflammatory mediators. Methods The ABI, IMT, Hs-CRP, IL-6 and blood pressure were measured in 48 newly diagnosed essential hypertension patients and 50 healthy volunteers. Results The carotid artery IMT, Hs-CRP, IL-6, systolic blood pressure and pulse pressure in hypertensive patients were significantly higher than those in healthy controls (p <0.05). ABI was significantly lower than that in healthy controls (p <0.05). ABI was negatively correlated with carotid IMT (r = 0.401), Hs-CRP (r = 0.549), IL-6 (r = 0.467), systolic blood pressure (r = 0.345) and pulse pressure (r = 0.354). Conclusions ABI, IMT and inflammatory mediators are of great significance in the evaluation of peripheral arterial stiffness in patients with essential hypertension. The ankle index is noninvasive, simple and suitable for high Blood pressure in the population as a routine screening tool to promote.
