目的探讨上皮钙黏蛋白(E-cadherin)和基质金属蛋白酶(MMP-2和MMP-9)在胃癌组织中的表达及临床意义。方法应用免疫组织化学SABC法和Western blot法检测60例原发性胃癌和15例正常胃黏膜组织E-cadherin和MMP-2、MMP-9的表达。结果胃癌组织中E-cadherin的阴性表达率为88.33%,显著高于正常胃黏膜组织的20.00%(P<0.01)。MMP-2和MMP-9在胃癌组织中的阳性表达率分别为67.39%和73.91%,高于正常胃黏膜组织的31.54%和21.47%(P<0.01)。E-cadherin的阴性表达率与胃癌的分化程度、淋巴结转移有关,而与患者年龄、性别、肿瘤大小及临床分期无显著相关性;MMP-2和MMP-9的阳性表达率与E-cadherin的阴性表达率呈正相关(r=0.453和r=0.579,P<0.01)。结论胃癌组织高表达MMP-2和MMP-9,同时低表达E-cadherin,提示E-cadherin和MMP-2、MMP-9的异常表达可能参与了胃癌的发生、发展过程。
Objective To investigate the expression and clinical significance of E-cadherin and matrix metalloproteinases (MMP-2 and MMP-9) in gastric cancer. Methods The expressions of E-cadherin, MMP-2, MMP-9 in 60 cases of primary gastric cancer and 15 cases of normal gastric mucosa were detected by immunohistochemical SABC and Western blot. Results The negative expression rate of E-cadherin in gastric cancer was 88.33%, which was significantly higher than that in normal gastric mucosa (20.00%, P <0.01). The positive rates of MMP-2 and MMP-9 in gastric cancer tissues were 67.39% and 73.91%, which were higher than those in normal gastric mucosa (31.54% and 21.47%, P <0.01). The negative expression rate of E-cadherin was correlated with the differentiation of gastric cancer and lymph node metastasis, but not with the age, sex, tumor size and clinical stage. The positive rate of MMP-2 and MMP-9 was positively correlated with the expression of E-cadherin Negative expression rate was positively correlated (r = 0.453 and r = 0.579, P <0.01). Conclusion The high expressions of MMP-2 and MMP-9 in gastric cancer tissues and the low expression of E-cadherin suggest that the abnormal expression of E-cadherin, MMP-2 and MMP-9 may be involved in the carcinogenesis and progression of gastric cancer.
