先天性喉喘鸣是婴儿的一种常见病,特征为吸气声粗糙及发作性喉鸣,典型者可见声门上区组织松弛,吸气时向中线靠拢。本病病理变化,一般认为系喉部解剖异常及声门上部气道软弱无力,如Ω形会厌,喉入口过度向前倾斜,吸气期杓状软骨及杓会厌襞向中线内移。据临床观察,先天性喉喘鸣的致病原因很可能是声门上区结构缺乏足够的弹性,而Ω形会厌、喉软骨发育未成熟似乎不是本症的重要病因。Kelemen按组织学观察指出,没有固有的声门上区扩张肌,但解剖10具成人尸体后提出茎突舌骨肌、茎突舌肌、茎突咽肌、舌骨舌肌、舌骨会厌韧带和舌内肌群等相互作用产生的力可使喉入口扩张。
