目的探讨强直性脊柱炎(ankylosing spondylitis,AS)的血脂谱特征及相关影响因素。方法回顾性纳入2005年1月—2015年1月60例AS患者及60名健康体检者。记录所有受试者的一般情况、临床特征以及炎症标志物水平,并检测总胆固醇(total cholesterol,TC)、三酰甘油(triacylglycerol,TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、极低密度脂蛋白胆固醇(very low-density lipoprotein cholesterol,VLDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)及C反应蛋白水平。结果 46.7%(28/60)的AS患者TG升高,而50.0%(30/60)的患者HDL-C下降。与健康对照组相比,AS患者TG、VLDL-C、VLDL-C/LDL-C、LDL-C/HDL-C及TC/HDL-C比值偏高,TC、LDL-C及HDL-C偏低,差异均有统计学意义(P<0.01);Spearman分析显示HDL-C水平与C反应蛋白呈负相关(rs=–0.359,P=0.005)。多因素分析显示HDL-C水平与C反应蛋白水平密切相关(P=0.019)。结论 AS血脂异常相当常见,主要特征是TG升高,HDL-C降低,并与炎症相关。这些结果提示AS患者动脉粥样硬化风险高。
Objective To investigate the features of serum lipids and the related factors in ankylosing spondylitis (AS). Methods Retrospectively included 60 AS patients and 60 healthy subjects from January 2005 to January 2015. The general conditions, clinical features, and levels of inflammatory markers in all subjects were recorded and the levels of total cholesterol (TC), triacylglycerol (TG), low-density lipoprotein cholesterol (LDL) -C), very low-density lipoprotein cholesterol (VLDL-C), high-density lipoprotein cholesterol (HDL-C) and C-reactive protein. Results 46.7% (28/60) of AS patients had elevated TG, while 50.0% (30/60) of patients had HDL-C decreased. Compared with healthy controls, patients with AS had higher TG, VLDL-C, VLDL-C / LDL-C, LDL-C / HDL-C and TC / HDL-C ratios, TC, LDL-C and HDL-C (P <0.01). Spearman analysis showed that the level of HDL-C was negatively correlated with C-reactive protein (rs = -0.359, P = 0.005). Multivariate analysis showed that HDL-C level was closely related to C-reactive protein (P = 0.019). Conclusions AS dyslipidemia is quite common, the main features are elevated TG, HDL-C decreased, and associated with inflammation. These results suggest that AS patients have a high risk of atherosclerosis.
