目的:评价体外循环中肺动脉灌注低温HTK液对法氏四联征患儿的肺保护作用。方法:法氏四联征患儿24例随机分为肺动脉灌注组和对照组各12例,检测手术前、后的氧合指数和肺泡-动脉氧分压差、白细胞介素-6、白细胞介素-8、可溶性细胞间黏附分子-1和P-选择素。结果:体外循环后各时间点,其灌注组氧合指数均明显高于对照组,2组间变化趋势差异有统计学意义;灌注组肺泡-动脉氧分压差均明显低于对照组;2组间体外循环后白细胞介素-6、可溶性细胞间黏附分子-1和P-选择素水平变化趋势差异有统计学意义,且灌注组体外循环后各时间点白细胞介素-8水平均较对照组明显降低。结论:体外循环中肺动脉灌注低温HTK液可有效减轻炎性反应的发生,提高法氏四联征矫治术后患儿肺换气功能,改善预后。
OBJECTIVE: To evaluate the pulmonary protective effect of hypothermic HTK solution during cardiopulmonary bypass on children with tetralogy of Fallot. Methods: Twenty-four children with tetralogy of Fallot were randomly divided into pulmonary arterial perfusion group (12 cases) and control group (12 cases). The oxygenation index and alveolar-arterial oxygen pressure difference before and after operation were measured. Interleukin-6 Su-8, soluble intercellular adhesion molecule-1 and P-selectin. Results: After cardiopulmonary bypass, the oxygenation index of perfusion group was significantly higher than that of the control group at each time point after CPB, and there was significant difference between the two groups in the change trend; the alveolar-arterial oxygen pressure difference in perfusion group was significantly lower than that in control group; 2 The changes of interleukin-6, soluble intercellular adhesion molecule-1 and P-selectin levels after cardiopulmonary bypass in the two groups were statistically different, and the levels of interleukin-8 in the perfusion group were higher than those in the control Group decreased significantly. CONCLUSION: Pulmonary arterial infusion of hypothermia HTK solution during cardiopulmonary bypass can effectively reduce the incidence of inflammatory reaction and improve the pulmonary ventilation function and prognosis in children with FAL.
