目的分析产超广谱β-内酰胺酶(ESBLs)鲍曼不动杆菌耐药性及耐消毒剂基因的研究,为医院控制产超广谱β-内酰胺酶(ESBLs)鲍曼不动杆菌传播提供依据。方法采用VITET 2 Compact仪器对临床分离100株鲍曼不动杆菌进行鉴定和药敏试验,采用K-B法进行ESBLs检测,采用PCR检测qac E△1、qac A/B、qac J、qac C、qac G耐消毒剂基因。结果 100株鲍曼不动杆菌中,有43株产ESBLs;在产ESBLs菌株中,检测有30株携带qac E△1基因,其他耐消毒剂基因检测为阴性。43株产ESBLs鲍曼不动杆菌对头孢哌酮/舒巴坦、美罗培南、亚胺培南的耐药率分别为69.7%、67.4%、69.7%,其他药物的耐药率均>90%。非产ESBLs鲍曼不动杆菌对常见抗生素的耐药率低于产ESBLs,差异有统计学意义(P<0.05)。结论产超广谱β-内酰胺酶鲍曼不动杆菌耐药率较高,其耐消毒剂基因以qacE△1为主。
OBJECTIVE: To study the drug resistance of Acinetobacter baumannii (ESBLs) -producing Acinetobacter baumannii and the disinfectant-resistant genes. It is an effective method to control the production of extended-spectrum β-lactamase Provide basis for dissemination. Methods The clinical isolates of 100 Acinetobacter baumannii were identified and susceptible tested by VITET 2 Compact instrument. The ESBLs were detected by KB method. The expression of qac A △ 1, qac A / B, qac J, qac C, qac G resistant to disinfectant genes. Results Among the 100 strains of Acinetobacter baumannii, 43 strains produced ESBLs. Among the ESBLs-producing strains, 30 strains were carrying qac E △ 1 gene and other anti-disinfectant genes were negative. The resistance rates of 43 ESBLs-producing Acinetobacter baumannii strains to cefoperazone / sulbactam, meropenem and imipenem were 69.7%, 67.4% and 69.7% respectively, and the drug resistance rates of other drugs were all> 90% . The rate of resistance to non-ESBLs-producing Acinetobacter baumannii to common antibiotics was lower than that of producing ESBLs (P <0.05). Conclusion Acinetobacter baumannii producing extended spectrum β-lactamase has a higher resistance rate, and its resistance to disinfectant genes is mainly qacE △ 1.
