目的:应用光学相干断层扫描(OCT)观察激光光凝治疗糖尿病黄斑水肿前后后极部视网膜厚度的变化,评价OCT在黄斑水肿治疗随访中的价值。方法:格栅样光凝治疗的患者24例(32眼)于治疗前和治疗后3个月行OCT检查,测量以黄斑中心凹为中心至4mm范围内视网膜厚度。结果:黄斑中心凹到2.2mm圆内激光治疗前及3个月术后视网膜神经上皮层厚度分别为635.6~400.6μm和378.2~180.6μm,两者比较有显著性差异(P<0.05);激光治疗前及3个月后黄斑中心凹到2.2~3.45mm视网膜神经上皮层厚度分别为459.1~229.2μm和447.2~155.4μm,两者比较无显著差异性(P>0.05)。结论:治疗3个月后黄斑区视网膜厚度降低,并且越接近黄斑中心凹处,降低作用越明显;OCT在黄斑水肿激光光凝治疗的随访观察中可发挥重要作用。
Objective: To observe the changes of retinal thickness before and after laser photocoagulation in diabetic macular edema with optical coherence tomography (OCT) to evaluate the value of OCT in the follow-up of macular edema. METHODS: Twenty-four patients (32 eyes) undergoing grid-like photocoagulation underwent OCT examinations before and 3 months after treatment, and the retinal thickness was measured with a foveal center to 4 mm. Results: The thickness of retinal neuroepithelium before and 3 months after laser treatment was 635.6-400.6μm and 378.2-180.6μm, respectively. There was a significant difference between the two (P <0.05). Laser There was no significant difference between the two groups (P> 0.05). The thickness of retinal nerve epithelium in the fovea between 2 and 3.45 mm were 459.1 ~ 229.2μm and 447.2 ~ 155.4μm respectively before treatment and 3 months after treatment. CONCLUSION: After 3 months of treatment, the retina thickness in macular area is decreased and the closer it is to the foveal of macula, the more obvious the effect of reduction is. OCT can play an important role in the follow-up observation of laser photocoagulation of macular edema.
