目的 :探讨经腰椎穿刺脑池气造影高分辨率CT(Gas HighresolutionCTcisternography ,Gas HRCTC)检查对内耳疾病的诊断价值。方法 :选择临床怀疑蜗神经瘤而常规增强前后CT扫描为阴性的患者 69例行Gas HRCTC检查。结果 :69例Gas HRCTC检查 ,局限于内耳道的占位性病变 7例 ,6例为小蜗神经瘤 ,1例为囊肿 ;Ⅶ Ⅷ脑神经血管袢压迫综合征 10例 ,蛛网膜炎 9例 ,43例表现正常。脑桥小脑角区的主要解剖结构为内耳道、脑桥小脑角 (Cerebellopontineangle ,CPA)池、面神经 前庭蜗神经 (Ⅶ Ⅷ )脑池段、小脑下前动脉袢、小脑绒球。内耳门前后径为 4.96± 0 .85mm ,Ⅶ Ⅷ脑神经脑池段神经束直径为 2 .2 8± 0 .41mm。结论 :Gas HRCTC检查能清晰显示脑桥小脑角区细微的解剖结构 ,对诊断小蜗神经瘤、蛛网膜炎、Ⅶ Ⅷ脑神经血管袢压迫综合征具有重要价值
Objective: To investigate the diagnostic value of lumbar puncture and gastroscopy high resolution computed tomography (Gas HRCTC) in the diagnosis of inner ear diseases. Methods: Gas HRCTC examination was performed in 69 patients with clinically suspected cochlear neuroma and routinely enhanced CT scan before and after CT scan. Results: Sixty-nine cases of Gas HRCTC were confined to 7 cases of space-occupying lesions in the internal auditory canal, 6 cases were microchina neuroma and 1 case was cyst. Among them, 10 cases were Ⅶ Ⅷ neurovascular 袢 compression syndrome and 9 cases were arachnoiditis , 43 cases showed normal. The main anatomic structures of the cerebellopontine angle region are internal auditory meatus, cerebellopontineangle (CPA) pool, parietal cochlear nerve (Ⅶ Ⅷ) cerebral cortex, anterior inferior cerebellar artery, and cerebellar pallidum. The anterior and posterior diameter of the inner ear door was 4.96 ± 0.85mm, and the diameter of the nerve bundle in the Ⅶ Ⅷ cerebral cortex was 2.28 ± 0.41mm. Conclusion: The Gas HRCTC examination can clearly show the fine anatomic structure of the cerebellopontine angle region, which is of great value in the diagnosis of microthyroid neuroma, arachnoiditis and Ⅶ Ⅷ neurovascular compression syndrome
