目的:研究人参总皂甙对心肌缺血和再灌注损伤的保护作用及浓度效应关系。方法:①11条杂种犬分二组,建立体外循环。人参总皂甙(GS)组心停搏液中加入GS20mg/L,测定心肌力学和血液动力学指标;取右室心肌标本电镜观察,线粒体记分。②30只大耳白兔分成5组,建立离体左心转流作功模型。GS组于心停搏液中加入GS20、40、80、160mg/L,测定心肌力学和血液动力学指标,同时取左心耳标本测定心肌内钙含量。结果:犬体外循环转流前后,兔离体左心转流作功前后,GS组与对照组比较,心肌力学指标±dp/dtmax,血流动力学指标CI(心脏指数),线粒体记分,心肌内钙含量,GS组均优于对照组(P<0.001),且GS80mg/L组优于GS20、40mg/L组(P<0.001),而GS160mg/L组上述指标均低于对照组(P<0.05)。结论:①GS20~80mg/L,对心肌缺血和再灌注损伤有明显的保护作用,且随浓度增加,作用增强。GS160mg/L,作用相反。②GS具有钙拮抗剂样效应
Objective: To study the protective effect of total saponins of Panax ginseng on myocardial ischemia and reperfusion injury and its concentration effect relationship. Methods: 111 mongrel dogs were divided into two groups to establish cardiopulmonary bypass. The ginsenoside (GS) group cardioplegia solution was added GS20mg/L, myocardial mechanics and hemodynamic parameters were measured; the right ventricular myocardial specimens were observed by electron microscopy and the mitochondria were scored. 230 big-eared white rabbits were divided into 5 groups to establish a work-out model of left heart flow. In the GS group, GS20, 40, 80, and 160 mg/L were added to the cardioplegia solution to determine the mechanical and hemodynamic parameters of the heart muscle. At the same time, the left atrial appendage specimen was taken to measure the intramyocardial calcium content. RESULTS: Before and after cardiopulmonary bypass in rabbits before and after cardiopulmonary bypass, the GS group was compared with the control group, the mechanical parameters of the heart were ±dp/dtmax, the hemodynamic parameters CI (cardiac index), mitochondrial score, and myocardium. The calcium content in the GS group was better than that in the control group (P<0.001), and the GS80mg/L group was better than the GS20 and 40mg/L groups (P<0.001), while the above indexes in the GS160mg/L group were lower than the GS80mg/L group. Control group (P<0.05). Conclusion: 1GS 20~80mg/L has obvious protective effect on myocardial ischemia and reperfusion injury, and the effect increases with the increase of concentration. GS160mg/L, the opposite effect. 2GS has a calcium antagonist-like effect
