目的:探讨c-myc基因检测对宫颈上皮内瘤样病变Ⅰ级(CINⅠ)自然转归的预测价值。方法:对病理组织学初次诊断为CINⅠ的患者43例进行不干预定期随访,采用荧光原位杂交技术检测c-myc基因以了解基线情况,随访6、12、18、24个月时分别行阴道镜病理组织学诊断。结果:43例患者失访3人,失访率6.98%,寿命表法得出平均随访41.3人,随访24个月后自然消退31例(75.1%),病变持续7例(16.9%),进展2例(4.8%)。c-myc阳性组与阴性组在随访12、18、24个月时,自然消退率、持续存在/进展率相比差异均有统计学意义(P<0.05)。结论:CINⅠ在2年内大多数能够自然消退,c-myc基因表达阳性者更易于病变持续和进展,可作为病变进展的辅助监测指标。
Objective: To investigate the predictive value of c-myc gene in the natural outcome of grade Ⅰ (CINⅠ) cervical intraepithelial neoplasia. Methods: Forty-three patients with primary pathologic diagnosis of CINⅠwere followed up for a period of non-intervention. Fluorescence in situ hybridization was used to detect the c-myc gene in order to understand the baseline conditions. During the follow-up period of 6, 12, 18 and 24 months, Microscopic histopathological diagnosis. Results: 43 patients were lost to follow-up, with a rate of loss of 6.98%. The average life expectancy was 41.3%. After 24 months of follow-up, 31 patients (75.1%) were spontaneous and 7 patients (16.9% 2 cases (4.8%). There was significant difference between spontaneous regression rate and persistence / progression rate in c-myc positive group and negative group at follow-up of 12, 18 and 24 months (P <0.05). CONCLUSION: Most of CIN I can regress spontaneously within 2 years. Patients with positive c-myc gene expression are more likely to sustain and progress the disease and may be used as secondary monitoring indicators for the progression of the disease.
