美国阴道镜检查和子宫颈病理学会(ASCCP)2019版指南中关于风险管理证据,加入了“阈值”的概念,主要依据既往筛查和此次筛查结果,评估当前和5年内子宫颈上皮内瘤变(CIN)Ⅲ及以上级别病变(CINⅢ+)的发生风险,并据此选择管理方案,包括5年随访、3年随访、1年随访、阴道镜检查、阴道镜检查或直接治疗、直接治疗.病理确诊的低级别鳞状上皮内病变[LSIL(CINⅠ)]和高级别鳞状上皮内病变[HSIL(CIN Ⅱ和Ⅲ)]的管理和随访,根据阴道镜检查前的细胞学风险以及此次筛查风险决定.既往HPV阴性、阴道镜活检病理检查未提示CIN Ⅱ及以上级别病变(CIN Ⅱ+)的妇女,CIN Ⅲ+的发生风险降低;既往HPV阳性、有CIN Ⅱ+或CIN Ⅲ+治疗史的妇女,CIN Ⅲ+的发生风险升高.
