急性淋巴细胞白血病(ALL)完全缓解率已明显提高,缓解期体内残留病灶(MRD)是导致复发而成为影响患者长期生存的主要因素。动态、定量监测ALL缓解期MDR水平具有独立的预后价值,有利于个体化治疗。本文复习了有关ALLMRD检测技术的进展,其中PCR检测白血病特异的DNA/RNA序列,如融合基因和IgH、TCR基因重排,敏感性10~(-4)～10~(-6);流式细胞术分析白血病细胞免疫表型敏感性10~(-4)～10~(-5)。MRD与预后的相关性得到一致肯定,但究竟何时何水平MRD临床价值最大尚需进一步研究确定。
The complete remission rate of acute lymphoblastic leukemia (ALL) has been significantly improved, remission residual disease (MRD) is the cause of recurrence and become a major factor affecting the long-term survival of patients. Dynamic and quantitative monitoring of ALL MDR levels with independent prognostic value, is conducive to individual treatment. In this review, we reviewed the progress of the ALLMRD detection technique, in which the leukemia-specific DNA / RNA sequences were detected by PCR, such as fusion gene and IgH, rearrangement of TCR genes, sensitivity of 10 -4 to 10 -6, Cytometry analysis leukemia cell immune phenotype sensitivity 10 ~ (-4) ~ 10 ~ (-5). The correlation between MRD and prognosis has been unanimously confirmed, but at what level and what level of MRD maximum clinical value needs further study to determine.
