目的探讨产前超声筛查及超声诊断在判断胎儿心脏畸形中的应用价值。方法选取2014年5月—2015年7月于成都市郫都区妇幼保健院做产科胎儿系统超声检查的孕妇5644例,对胎儿心脏切面进行分析,诊断心脏畸形的情况。结果 5644例孕妇中,产前诊断胎儿心脏畸形22例,漏诊3例,漏诊率为0.47%,各切面检出率均在70.0%以上,单一四腔心切面检出胎儿为心脏畸形的检出率为81.82%,四腔心切面联合流出道切面的检出率为90.91%。对产前诊断胎儿心脏畸形孕妇进行针对性处理,并经进一步检查与产前诊断结果一致。结论在胎儿产前超声诊断过程中,产科医生除需掌握四腔心切面外,还应掌握其他切面的判定,以提升心脏畸形产前检出率。
Objective To investigate the value of prenatal ultrasound screening and ultrasonography in the diagnosis of fetal heart deformity. Methods From May 2014 to July 2015, 5644 pregnant women who underwent ultrasound examination of obstetric fetal system in Chengdu Qidu District Maternal and Child Health Hospital were selected to analyze the fetal heart section to diagnose the heart malformation. Results Among the 5644 pregnant women, 22 cases of prenatal diagnosis of fetal heart malformations and 3 cases of missed diagnosis were misdiagnosis rates of 0.47% and the detection rates of all the sections were above 70.0%. The fetus was diagnosed as cardiac malformation The rate of exit was 81.82%. The detection rate of the four-chamber coronal combined outflow tract surface was 90.91%. Prenatal diagnosis of fetal heart malformations targeted treatment, and after further examination and prenatal diagnosis consistent. Conclusion In the process of prenatal ultrasound diagnosis, the obstetrician should master the judgment of other aspects in addition to the four-chamber heart-shaped section so as to improve the prenatal detection rate of cardiac malformation.
