目的探讨喉癌CT灌注参数与血管内皮生长因子(VEGF)表达的相关性及其在喉癌临床诊治中的应用价值。方法选取经临床活检证实的喉癌病例23例,行术前CT灌注检查并计算各灌注参数(BF、BV、MTT、PS)值,术后取病变组织行病理学检查证实并行免疫组化染色,检测其VEGF积分。应用SPSS 13.0统计软件包分别对喉癌组织和正常肌肉组CT灌注参数以及喉癌组织CT灌注参数和喉癌术后组织血管内皮生长因子积分进行统计学处理,统计分析各CT灌注参数值与VEGF积分的相关性。结果喉癌组织和相应区域正常肌肉间各灌注参数BF值、BV值、MTT值及PS值均存在显著性差异,喉癌的灌注参数明显高于正常肌肉(P<0.05)。喉癌的灌注参数的BF值和BV值与其VEGF积分呈正相关(r=0.502,0.327,P<0.05),MTT值与其VEGF积分呈负相关(r=-0.511,P<0.05),而PS值与其VEGF积分无相关性(r=0.275,P>0.05)。结论喉癌CT灌注参数BV、BF值与其VEGF积分具有明显的相关性,可以在一定程度上反映肿瘤的血供情况,术前通过CT灌注成像观察其参数值可以明显提高术前喉癌检查的准确性以及对指导临床抗肿瘤血管生成治疗有重要的意义,具有较高的临床应用价值。
Objective To investigate the correlation between CT perfusion parameters and the expression of vascular endothelial growth factor (VEGF) in laryngeal carcinoma and its clinical value in the diagnosis and treatment of laryngeal cancer. Methods Twenty-three cases of laryngeal carcinoma confirmed by clinical biopsy were selected. CT perfusion examination and perfusion parameters (BF, BV, MTT, PS) were performed before operation. Pathological examination was performed to confirm the parallel immunohistochemical staining , Detected VEGF score. SPSS 13.0 statistical package were used to calculate the CT perfusion parameters of CT and CT perfusion parameters of laryngeal carcinoma tissue and vascular endothelial growth factor after laryngeal cancer respectively. The BF and BV values of perfusion parameters of laryngeal carcinoma were positively correlated with their VEGF scores (r = 0.502,0.327, P <0.05), and the MTT values were negatively correlated with their VEGF scores (r = -0.511, P <0.05) There was no correlation with VEGF score (r = 0.275, P> 0.05). Conclusion The CT perfusion parameters BV and BF of laryngeal carcinoma have significant correlation with VEGF score, which can reflect the blood supply of tumor to some extent. The preoperative CT perfusion imaging can obviously improve the parameters of preoperative laryngeal cancer examination Accuracy and guidance of clinical anti-angiogenesis therapy has important significance, with high clinical value.
