纳洛酮(Naloxone),化学名为烯丙羟吗啡酮,即1-N-烯丙羟-去甲二氢吗啡酮,商品名Narcan,为特异性阿片受体拮抗剂,近年来,临床与实验证实纳洛酮具有良好的抗休克效果,且安全性高、副作用小,引起了医学界的关注。一、临床药理 1972年Collier等提出动物脑中有能与阿片受体结合的内源性吗啡样物质(OLS),其中最强有力的是β-内啡肽。机体应激时,下丘脑释放因子促使垂体前叶释放AC-TH和β-内啡肽增加,曾有报告休克时,血中内啡肽浓度可升高5—6倍。1978年Holaday首次报告垂体分泌的内啡肽可致低血压休克,β-内啡肽抑制前列腺素和儿茶酚
