目的:为了解在慢性阻塞性肺病(COPD)中尘螨过敏的发生状况,以及尘螨过敏对急性加重发生的作用。方法:采用常用的过敏参数和检测手段,对149例住院的COPD、支气管哮喘和非呼吸系统疾病的患者进行分组检测,其中COPD59例,支气管哮喘30例,非呼吸系统疾病患者60例。测定痰找嗜酸细胞(EOS)、外周血嗜酸细胞计数、血清IgE和屋尘螨(HDM)皮肤过敏试验。同时进行居住条件、不良卫生习惯和软家具设施调查。结果:与非呼吸系统疾病患者相比,COPD患者痰找EOS增加,外周血EOS计数增加,血清IgE增高和皮肤试验阳性率均有显著差异;而与支气管哮喘组相比,痰找EOS及皮肤试验阳性率无差异(P>005),虽COPD组外周血EOS计数和IgE浓度有增高比支气管哮喘者仍有差异(P<0.01)。COPD皮试阳性组较阴性组两年内急性加重次数明显增多。结论:COPD患者存在着HDM暴露与过敏,HDM过敏对于COPD患者急性加重的作用应值得重视;COPD患者HDM过敏的原因可能与居住条件不良和卫生习惯不良有关
Objective: To understand the occurrence of dust mite allergy in chronic obstructive pulmonary disease (COPD) and the role of dust mite allergy in acute exacerbations. Methods: A total of 149 inpatients with COPD, bronchial asthma and non-respiratory diseases were divided into groups by commonly used allergy parameters and testing methods. Among them, 59 were COPD, 30 were bronchial asthma and 60 were non-respiratory diseases. Phlegm finding eosinophils (EOS), peripheral blood eosinophil counts, serum IgE and house dust mite (HDM) skin allergy tests. At the same time living conditions, poor health habits and soft furniture facilities survey. Results: Compared with patients with non-respiratory diseases, patients with COPD had higher EOS, increased counts of EOS in peripheral blood, increased serum IgE and positive rates of skin test. Compared with bronchial asthma group, sputum showed EOS and skin There was no difference between the two groups (P> 005). Although the EOS count and IgE concentration in COPD group were still higher than those in bronchial asthma group (P <0.01). COPD skin test positive group than in the negative group within two years significantly increased the number of acute exacerbation. Conclusions: HDM exposure and allergy exist in patients with COPD. HDM allergy should be emphasized in patients with acute exacerbation of COPD. The causes of HDM allergy in COPD patients may be related to poor living conditions and poor hygiene habits
