心房扑动时,听到心房音并录下图形的报告是很少的,至今在文献中大约仅有20例。近来我们遇到一例非常典型。一位男性凤心病患者发生了心房扑动,心电图证示房扑,F 波率为300次/min,呈2:1房室传导。于胸骨左缘可闻频速、高调清脆的喀喇样的附加心音,第三四肋间隙最清晰。心音图有下列特点:(1)心房音(Sa)与房扑波(F)密切相关,F—Sa 间期为0.19秒;(2)Sa—Sa 间期与 F—F 间期完全相等,频率为300次/分;(3)Sa 在心室舒张期明显,Sa>1/3S_1;(4)Sa 的出现和消失与 F 波的发生和消失完全一致;可见 Sa 是心房扑动的心房音。本文讨论了心房扑动心房音发生机制,我们认为心房扑动心房音产生机制与第四心音不同,Sa 可能起源于心房。
