目的:探讨面神经失神经支配程度与预后的关系及其机理.方法:应用神经兴奋性测试仪测试失神经支配程度.制作豚鼠颞骨内面神经麻痹(FP)模型,观察面肌功能恢复时间.透射电镜观察面神经的病理改变.结果:190例FP由恢复到临床治愈,失神经支配阴性组需(33.1±21.3)D;失神经支配Ⅰ度组需(63.3±12.4)D;失神经支配Ⅱ度组需(90.1±11.7)D;失神经支配Ⅲ度组需(210.2±37.5)D;4组间差异显著.压迫面神经5S,神经兴奋性传导恢复时间和瞬目反射恢复时间都是(60.7±45.3)H;15S组神经兴奋性传导恢复时间(982.6±36.6)H,瞬目反射恢复需(46±12)D;30S组神经兴奋性传导和瞬目反射无1只恢复到正常.电镜观察5S和15S组髓鞘板层开离,30S组髓鞘融解轴突破坏.结论:失神经支配程度是预后诊断的指标,并与髓鞘和轴突破坏程度有关.“,”Objective: Our aim was to study the correlationship between denervation degree and prognosis in facial palsy. Methods: Denervation degree was measured with nerve excitability measurement apparatus. By making 43 cases of guinea pig model of intratemporal facial palsy, we studied the restorative period of facial muscle, observed the pathological change of guinea pigs' facial nerve by transmission electron microscope. Results: The time which it took to cure the guinea pigs with facial palsy was (33.1±21.3) days for the negative group; (63.3±12.4) days for the positive group; (90.1±11.7) days for the intense positive group; (210.2±37.5) days for the super intense positive group. There was a significant difference among the four groups. For the group with compressing test of the facial nerve for 15 seconds, the repressive period of nerve excitability conduction was (60.7±45.3) h. For the group with compressing for 15 seconds, it took (46±12) d to recover the blink reflex. The blink reflex could not be restored for 30 seconds group. Mild separation of myelin sheath was observed in 5 seconds and 15 seconds group, destruction of myelin sheath and axon was observed in 30 seconds group. Conclusion: Measuring the nerve excitability was a useful method to judge the degree of denervation and was of value for the prognosis. There was a relationship between devervation degree and the destructive degree of myelin sheath and axon.
