目的观察苯妥英钠血浓度与神经毒性的关系。方法用HPLC法测定苯妥英钠血浓度。结果出现苯妥英钠神经毒性反应者的血浓度为15~51μg/ml,平均为30±8.2μg/ml。诱导出眼球震颤,共济失调及精神症状的血浓度分别为22±4.1μg/ml,30±8.2μg/ml及47±4.0μg/ml。结论四分之一患者在相对安全的有效血浓度范围内出现药物不良反应。调整苯妥英钠剂量应据其临床发作情况,药物不良及血浓度综合分析。
Objective To observe the relationship between the concentration of phenytoin sodium and neurotoxicity. Methods HPLC method for the determination of phenytoin sodium blood concentration. The results showed phenytoin sodium toxicity of blood concentration of 15 ~ 51μg / ml, with an average of 30 ± 8.2μg / ml. The blood concentrations that induced nystagmus, ataxia, and psychiatric symptoms were 22 ± 4.1 μg / ml, 30 ± 8.2 μg / ml, and 47 ± 4.0 μg / ml, respectively. Conclusion One quarter of patients experienced adverse drug reactions in the relatively safe and effective blood concentration range. Adjustment of phenytoin dose should be based on clinical seizures, poor drug and blood concentration of a comprehensive analysis.
