目的对深圳市剖宫产妇及其新生儿血清中HBV标记物与麻疹抗体进行评价,以了解该市人群HBV感染现状和麻疹免疫水平。方法应用横断面流行病学研究方法,对205名剖宫产妇及其新生儿与318名对照人群进行了调查。用酶联免疫吸附试验检测血清中的HBsAg、抗-HBc、抗-HBs和麻疹lgG抗体。结果剖宫产妇中的HBsAg流行率(13.17%)、HBV流行率(82.93%)和HBV感染率(56.59%)均显著性地高于对照人群(5.35%、68.55%和42.77%);而剖宫产妇中的HBV标记阴性率(17.07%)和麻疹抗体的阳性率(64.39%)低于对照人群(31.45%和72.01%)。新生儿中有着极低的HBsAg流行率(0.49%)。麻疹抗体剖宫产妇阴性但新生儿阳性检出模式的比率为12.20%。剖宫产妇中接种乙型肝炎疫苗者单一抗-HBs阳性率(32.77%)显著高于未接种疫苗者(12.82%)和接种史不详者(21.28%)。结论该市中的剖宫产妇是HBsAg携带的高危险人群,同时也是具有高比例对麻疹病毒易感的人群。因此,制定对该市中未婚女青年易感人群的乙型肝炎疫苗预防接种和麻苗强化免疫措施及提前对新生儿开展麻疹疫苗初免策略,对于控制孕产妇HBV感染和未到8月龄儿童发生麻疹,达到消除麻疹目标是非常重要的。
Objective To evaluate the serum levels of HBV markers and measles among cesarean section women and their newborns in Shenzhen to find out the status of HBV infection and measles immunity in the city. Methods A total of 205 cesarean section women, their newborns and 318 control subjects were investigated using a cross-sectional epidemiological study. Serum HBsAg, anti-HBc, anti-HBs and measles lgG antibodies were detected by enzyme-linked immunosorbent assay. Results The prevalence of HBsAg (13.17%), HBV prevalence (82.93%) and HBV infection (56.59%) in cesarean section women were significantly higher than those in control group (5.35%, 68.55% and 42.77% The negative rate of HBV markers (17.07%) and the positive rate of measles antibody (64.39%) in cesarean section women were lower than those in control group (31.45% and 72.01%). There is a very low prevalence of HBsAg in newborns (0.49%). The ratio of measles antibody cesarean section negative but neonatal positive detection mode was 12.20%. The positive rate of single anti-HBs (32.77%) was significantly higher in unvaccinated women than in unvaccinated persons (12.82%) and those with unknown history of vaccination (21.28%). Conclusion The cesarean section in the city is a high-risk population of HBsAg carriers and also has a high proportion of people susceptible to measles virus. Therefore, the development of the city’s unmarried young women susceptible population of hepatitis B vaccine immunization and measles vaccine and early immunization measles vaccine for neonatal strategy to control maternal HBV infection and not yet reached 8 months of age It is very important for measles to be eradicated in measles.
