根治性颈清扫术后产生的颅高压症 ,预防的关键在于切除颈内静脉前要有一种有助于判断侧支代偿的方法 .本研究随机选择 33例颈清扫患者 ,对颈内静脉结扎前 JVP ,结扎后 JVPs 的静脉压力测定 ,结合颈内静脉结扎前后的管径测量 .结果 :1.JVP和 JVPs平均值分别是 10 .2 4和 30 .39mm Hg,P<0 .0 1;2 .右侧 19例分别是11.74和 4 2 .79mm Hg,P<0 .0 1;3.左侧 14例分别是 8.2 1和 13.57,P>0 .0 5;4 .右、左两侧 JVP~JVPs平均升高31.2 3和 5.36 ,P<0 .0 1.表明 :颈内静脉结扎前后静脉压的升高是有临床意义 ;同时提示右侧可能是功能侧 ;颅高压与 JVPs有关
The key to prevention of cranial hypertension after radical neck dissection is a method that is helpful in judging collateral compensation before resection of the internal jugular vein. This study randomly selected 33 patients with neck dissection and ligating the internal jugular vein. For the former JVP, the venous pressure of the JVPs after ligation was measured, combined with the measurement of the diameter of the internal jugular vein before and after ligation. Results: 1. The mean values of JVP and JVPs were 10.24 and 30.39 mm Hg, respectively, P<0.01; 2. The right side of the 19 cases were 11.74 and 42.79 mm Hg, respectively; P<0. 0 1; 3. The left side of the 14 cases were 8.2 1 and 13.57, respectively, P> 0. 0 5; 4. Right and left sides The average increase in JVP-JVPs was 31.2 3 and 5.36. P<0 1. 0 indicates that the increase of venous pressure before and after the ligation of the internal jugular vein is clinically significant; at the same time, the right side may be indicated as a functional side; intracranial hypertension is associated with JVPs.
