用体外循环作冠状动脉手术之后所出现的视力丧失,是由于栓塞或脑血流灌注降低所引起的视放射或扰叶梗死而造成。视力丧失和梗死引起的视神经头苍白水肿,即缺血性视神经疼(Ischemic Optic Neuropathy.,10N)现将此症首次加以描述。作者在两个机构中进行了7685例冠状动脉疾病的搭桥手术,其中7例发生了10N,全为男性,年龄为40~61岁,每例均有视神经盘苍白肿胀,但没有视网膜梗塞。视野缺损为双侧者3例,右侧3例,左侧1例。视野丧失的性质呈典型“水平性”(altitudinal),全部病例没有其他系统的栓塞,血沉、纤维蛋白元水平均正常,也没有巨细胞性动脉炎的证据。7例中6例进行了头颅CT扫描,5例正常,1例有右顶颞叶区梗死。
Loss of vision after cardiopulmonary bypass with cardiopulmonary bypass is caused by visual radiation or infarct infarction caused by reduced embolization or cerebral perfusion. Visual loss and infarction caused by pale head edema, namely, ischemic optic neuropathy (Ischemic Optic Neuropathy., 10N) is the first time this disease is described. The authors conducted 7,685 coronary artery bypass grafting operations in two facilities, 7 of which occurred in 10N, all men, aged 40-61 years. Each had a pale swelling of the optic disc but no retinal infarction. Visual field defects were bilateral in 3 cases, right in 3 cases, left in 1 case. The nature of loss of vision was typically “altitudinal,” with no other systemic embolism, erythrocyte sedimentation, fibrinogen levels, and no evidence of giant cell arteritis in all cases. Six of the seven patients underwent CT scans, five were normal, and one had a right-sided temporal lobe infarction.
