目的评价国产机械吻合器在食管癌和贲门癌手术中的应用。方法选取自2002年10月至2008年8月期间,采用国产圆形管状吻合器行食管、贲门癌切除吻合术158例,其中男103例,女55例,年龄44～79岁。食管癌140例,贲门癌18例。食管胃颈部吻合7例,胸顶吻合81例,弓上吻合43例,弓下吻合27例。结果术后出现吻合口瘘3例(1.9%),吻合口狭窄6例(3.8%),吻合口出血2例(1.2%),术中吻合出现机械故障2例(1.2%)。结论国产机械吻合器在食管贲门癌手术中是安全省时的,可反复使用,价格低廉,能满足基层医院和偏远地区大部分患者的需要。
Objective To evaluate the application of domestic mechanical staplers in esophageal and cardiac surgery. Methods From October 2002 to August 2008, a total of 158 cases of esophageal and gastric cardia resection and anastomosis were performed with domestic round tubular staplers, including 103 males and 55 females, aged 44-79 years. 140 cases of esophageal cancer, cardia cancer in 18 cases. Esophagogastric stomach anastomosis in 7 cases, chest anastomosis in 81 cases, bow on the anastomosis in 43 cases, bow under the anastomosis in 27 cases. Results There were 3 cases (1.9%) of anastomotic fistula, 6 cases of anastomotic stenosis (3.8%) and 2 cases of anastomotic bleeding (1.2%). There were 2 cases of mechanical anastomosis (1.2%) during operation. Conclusion The domestic mechanical stapler is safe and time-saving in the operation of esophageal and cardia cancer, which can be used repeatedly and at low cost, which can meet the needs of most hospitals in primary hospitals and remote areas.
