目的探讨T淋巴细胞CD28表达与乙肝病毒携带(ASC)者免疫耐受的关系。方法采用流式细胞仪定量检测ASC者外周血及肝组织中CD28的水平;采用免疫组化技术检测ASC者肝组织CD28的表达率。结果不同Knodell积分的ASC者肝组织T淋巴细胞CD28表达情况间差别有显著性意义(P<0.05),ASC者中83.9%为无表达或低表达。ASC者外周血及肝组织中CD8+/CD2+8表达率分别为(20.0±5.8)%和(19.0±6.8)%,均较正常组低(P<0.05),且肝组织中CD8+/CD2+8水平较外周血下降更明显,差别有显著性意义(P<0.01)。肝组织中CD8+/CD2+8表达率随着肝组织学积分的减少而下降。结论ASC者肝组织CD28表达明显低下,T淋巴细胞不能完全活化,处于相对免疫耐受状态。ASC者肝组织中CD8+/CD2+8表达率较外周血更能反映肝脏免疫耐受状态。T淋巴细胞CD28表达与ASC者免疫耐受关系密切,且在其免疫耐受发病机制中起着重要作用。
Objective To investigate the relationship between CD28 expression in T lymphocytes and immune tolerance of hepatitis B virus carriers (ASC). Methods The levels of CD28 in peripheral blood and liver of ASC were detected by flow cytometry. The expression of CD28 in ASC was detected by immunohistochemistry. Results There was a significant difference in the expression of CD28 between T lymphocytes of ASC patients with different Knodell scores (P <0.05), and 83.9% of ASC patients had no expression or low expression. The expression rates of CD8 + / CD2 + 8 in peripheral blood and liver of ASC were (20.0 ± 5.8)% and (19.0 ± 6.8)%, respectively, lower than those in normal group (P <0.05) 8 levels more significantly lower than the peripheral blood, the difference was significant (P <0.01). The expression of CD8 + / CD2 + 8 in liver tissue decreased with the decrease of liver histological score. Conclusion The expression of CD28 in ASC liver tissue is obviously low, and T lymphocyte can not be completely activated, which is in a state of relative immune tolerance. The expression of CD8 + / CD2 + 8 in ASC liver tissue can reflect the state of liver immune tolerance better than the peripheral blood. The expression of CD28 on T lymphocytes is closely related to immune tolerance in ASC and plays an important role in the pathogenesis of immune tolerance.
