目的探讨老年原发性高血压患者静息心率(RHR)变化与靶器官损害的关系。方法162例老年原发性高血压患者按RHR值分为3组,RHR<65次/min为1组(54例),65次/min≤RHR<80次/min为2组(58例),RHR≥80次/min为3组(50例)。对入选患者测定RHR、血糖、血脂、体质指数、肌酐、尿酸,行心电图、超声心动图、颈动脉超声检查。结果随着RHR的增高,总胆固醇、甘油三酯、低密度脂蛋白胆固醇、尿酸、尿微量白蛋白有上升趋势;左室质量指数、颈总动脉内膜厚度增加,左室射血分数下降、颈动脉内径增大,各组之间比较差异有统计学意义(P<0.05或<0.01)。结论RHR增高与老年高血压靶器官损害有密切关系,在控制血压的同时应注重心率的控制。
Objective To investigate the relationship between resting heart rate (RHR) and target organ damage in elderly patients with essential hypertension. Methods A total of 162 elderly patients with essential hypertension were divided into three groups according to the RHR value: RHR <65 beats / min in one group (54 cases), 65 times / min≤RHR <80 beats / min in two groups (58 cases) , RHR≥80 beats / min for 3 groups (50 cases). Selected patients were measured RHR, blood glucose, blood lipids, body mass index, creatinine, uric acid, ECG, echocardiography, carotid ultrasound. Results With the increase of RHR, total cholesterol, triglyceride, low density lipoprotein cholesterol, uric acid and urine microalbumin increased. Left ventricular mass index, carotid artery intima thickness increased, left ventricular ejection fraction decreased, Carotid artery diameter increased, the difference between the groups was statistically significant (P <0.05 or <0.01). Conclusions The increase of RHR is closely related to the target organ damage in elderly hypertensive patients. In controlling blood pressure, we should pay attention to the control of heart rate.
