目的:探索城区居家高龄老人健康服务模式。方法:以项目为试点,构建开展居家高龄老人健康服务的组织管理网络、服务内容、信息化体系、绩效考评。结果:社区居家养老服务品质提升,老年人健康管理服务深入,社区卫生服务内涵深化。结论:建立政府主导、绩效优先、医护参与、智慧医养的健康服务模式,促进高龄老人医养结合事业可持续发展。
Objective: To explore the urban elderly service model of elderly health services. Methods: Taking the project as a pilot project, we constructed an organization and management network, service contents, information system and performance appraisal to carry out the health services for elderly people at home. Results: The quality of community home care services was improved, the health care services for the elderly were deepened, and the connotation of community health services was deepened. Conclusion: The establishment of a government-led, performance-based, health care participation, health care and wisdom of the health service model to promote the elderly with medical care combined with the cause of sustainable development.
