目的:探讨超早期肠内营养支持护理对重症颅脑损伤(SHI)患者营养状态、康复进程的影响。方法:选择解放军海军安庆医院重症医学科2018年1月—2021年1月收治的112例SHI患者,按照随机数字表法分为超早期组与对照组,各56例。超早期组接受超早期肠内营养支持护理(手术后6 h内给予早期肠内营养支持及护理),对照组接受常规早期肠内营养支持护理(手术后24 h内给予肠内营养支持及护理)。比较两组患者的营养指标如血清血红蛋白(Hb)、血清总蛋白(TP)、白细胞计数(WBC)、淋巴细胞计数(TLC)、转铁蛋白(TF)、氮平衡(NB)水平以及肛门排便排气时间、肠鸣音恢复时间,并评估两组的术后并发症发生情况。结果:护理后,两组患者的血清Hb、TP、WBC、TLC、TF、NB水平均高于护理前,超早期组患者的血清Hb、TP、WBC、TLC、NB水平高于对照组,差异有统计学意义(n P<0.05)。超早期组患者的肛门排便排气时间、肠鸣音恢复时间均短于对照组,差异有统计学意义(n P<0.05)。超早期组患者的术后感染、多器官损害、反复消化道出血例数均低于对照组,差异有统计学意义(n P<0.05)。n 结论:超早期肠内营养支持护理可以改善SHI患者的营养水平,同时降低术后并发症发生率,有利于颅脑损伤患者的康复。“,”Objective:To explore the effect of super early enteral nutrition support nursing on nutritional status and rehabilitation process of patients with severe head injury (SHI) .Methods:A total of 112 SHI patients admitted to Department of Critical Care Medicine of the PLA Navy Anqing Hospital from January 2018 to January 2021 were selected. According to random number table method, they were divided into the super early group and the control group, with 56 cases in each group. The super early group received super early enteral nutrition support nursing (early enteral nutrition support and nursing within 6 h after operation) , while the control group received routine early enteral nutrition support nursing (enteral nutrition support and nursing within 24 h after operation) . Nutritional indicators, such as serum hemoglobin (Hb) , serum total protein (TP) , total white blood cell (WBC) , total lymphocyte count (TLC) , transferrin (TF) , nitrogen balance (NB) levels and anal defecation exhaust time and bowel sound recovery time were compared between the two groups, and the incidence of postoperative complications were evaluated in the two groups.Results:After nursing, the levels of serum Hb, TP, WBC, TLC, TF and NB in two groups were higher than those before nursing, and the levels of serum Hb, TP, WBC, TLC and NB in the super early group were higher than those in the control group, and the differences were statistically significant (n P<0.05) . The recovery time of anus defecation and bowel sound in the super early group were shorter than those in the control group, and the differences were statistically significant (n P<0.05) . The incidences of postoperative infection, multiple organ damage and repeated gastrointestinal bleeding in the super early group were lower than those in the control group, and the differences were statistically significant (n P<0.05) .n Conclusions:Super early enteral nutrition support nursing can improve the nutritional level of SHI patients and reduce the incidence of postoperative complications, which is conducive to the rehabilitation of patients with head injury.
