目的比较端粒酶检测和脱落细胞学检查在胸、腹水诊断中的作用。方法用 PCR-TRAP法检测 65例胸腹水标本中端粒酶的表达 ,同时做脱落细胞学检查。结果在 65例胸腹水中 ,端粒酶检测和细胞学检查的符合率为 84.6% (5 5 /65 ) ,两者的诊断结果具有一致性 (P<0 .0 1) ;在有原发恶性肿瘤病灶的 43例中 ,两种方法的阳性率无显著性差异 (P>0 .0 5 ) ;在无原发恶性肿瘤病灶的 2 2例中 ,端粒酶检测的特异性为 90 .9% (2 0 /2 2 )。结论端粒酶检测是胸腹水良、恶性鉴别的良好方法 ,与细胞学检查有互补性。
Objective To compare the role of telomerase and exfoliative cytology in the diagnosis of chest and ascites. Methods The expression of telomerase in 65 samples of pleural effusion and ascitic fluid was detected by PCR-TRAP, and exfoliated cytology was performed at the same time. Results In 65 cases of pleural effusion and ascites, the coincidence rate of telomerase and cytology was 84.6% (55/65), the diagnostic results were consistent (P <0.01) There were no significant differences in the positive rates of the two methods among the 43 cases of malignant tumor (P> 0.05). The specificity of telomerase detection was 90 in 22 cases without malignant tumor lesions. 9% (20/2 2). Conclusion Telomerase detection is a good method for the differential diagnosis of malignant ascites and pleural effusions, which is complementary to cytological examination.
