目的探讨64层螺旋CT扫描在颌骨肿瘤诊断中的应用。方法选择2011年2月至2013年2月间收治的颌骨肿瘤患者120例(良性肿瘤72例,恶性肿瘤48例)进行螺旋CT扫描与三维CT重建。结果良性肿瘤轮廓清楚,内部密度不均匀,骨质膨胀性,无软组织肿块。恶性肿瘤轮廓不清楚,内部密度均匀,骨质吸收破坏,有软组织肿块。恶性肿瘤的CT差异值明显高于良性肿瘤,差异有统计学意义(P<0.05)。多平面重建(MPR)与容积重建(VR)图像对颌骨肿瘤累及上颌窦的显示差异无统计学意义(P>0.05),而对累及鼻道鼻腔和累及牙槽骨的显示差异有统计学意义(P<0.05)。结论 64层螺旋CT在颌骨肿瘤扫描中能获得特异的图像,对于不同肿瘤有很好的鉴别价值,应用CT重建技术有利于指导手术治疗。
Objective To explore the application of 64-slice spiral CT in the diagnosis of jaw tumors. Methods From February 2011 to February 2013, 120 patients with maxillary tumors (72 benign tumors and 48 malignant tumors) were selected for spiral CT and three-dimensional CT reconstruction. Results Clear outline of benign tumors, uneven internal density, bone expansion, no soft tissue mass. Malignant tumor contour is not clear, the internal density, bone destruction, with soft tissue mass. The CT difference of malignant tumors was significantly higher than that of benign tumors, the difference was statistically significant (P <0.05). MPR and VR images showed no significant difference in maxillary sinus involvement between maxillary and mandibular tumors (P> 0.05), but statistically significant differences were found in involvement of nasal passages and alveolar bone Significance (P <0.05). Conclusion 64-slice spiral CT can obtain specific images in the jaw tumor scanning. It has good differential value for different tumors. It is helpful to guide the surgical treatment with CT reconstruction.
