目的:探讨彩色多普勒超声在糖尿病眼部血管血流动力学检测中的应用价值。方法:纳入本院2013年5月1日到2014年11月30日收治的2型糖尿病眼病患者180例,通过眼底镜检查,将180例2型糖尿病眼病患者分为3组,糖尿病无视网膜病变研究组(NDR组)67例、糖尿病视网膜病变非增殖期组(NPDR组)52例、糖尿病视网膜病变增殖期组(PDR组)61例,并选取健康人群30人作为对照组,对纳入分析的患者和健康人群进行彩色多普勒超声检查并对视网膜中央动脉(CRA)、睫状后动脉(PCA)、眼动脉(OA)的血流参数指标分析。结果:2型糖尿病眼病患者CRA、PCA和OA的收缩期峰值流速(PSV)、舒张末期血流速度(EDV)均较健康人群低(P<0.05);阻力指数RI)均较健康人群高(P<0.05);其中CRA、PCA和OA在NDR组、NPDR组和PDR组的PSV、EDV呈下降趋势,组间比较差异有统计学意义(P<0.05);RI呈上升趋势,组间比较差异有统计学意义(P<0.05)。结论:彩色多普勒超声监测眼部血管的血流状态可以对2型糖尿病眼病的病变严重程度进行判定,从而为该病的治疗和疾病预后的判断提供有价值的依据。
Objective: To investigate the value of color Doppler ultrasonography in detecting ocular hemodynamics in diabetic patients. Methods: One hundred and eighty patients with type 2 diabetic ophthalmopathy who were admitted to our hospital from May 1, 2013 to November 30, 2014 were divided into three groups according to the ophthalmoscopy. 180 patients with type 2 diabetes mellitus were divided into three groups. Diabetic patients without retinopathy The study group (NDR group) 67 cases, diabetic retinopathy non-proliferative group (NPDR group) 52 cases, diabetic retinopathy proliferative phase group (PDR group) 61 cases, and healthy people 30 were selected as the control group, Color Doppler echocardiography was performed in patients and healthy subjects and blood flow parameters were analyzed for central retinal artery (CRA), posterior ciliary artery (PCA) and ophthalmic artery (OA). Results: The peak systolic velocity (PSV) and end diastolic velocity (EDV) of CRA, PCA and OA in patients with type 2 diabetes were lower than those in healthy people (P <0.05), while the resistance index (RI) P <0.05). The levels of PSV and EDV of CRA, PCA and OA in NDR group, NPDR group and PDR group showed a decreasing trend, the differences between the two groups were statistically significant (P <0.05); RI showed an upward trend; The difference was statistically significant (P <0.05). Conclusion: Color Doppler ultrasound blood flow monitoring of the eye can determine the severity of type 2 diabetic eye disease, which provides a valuable basis for the treatment of the disease and the prognosis of the disease.
