目的 建立高效、快速的外周血中细胞角蛋白 2 0 (Cytokeratin 2 0 ,CK2 0 )表达的检测方法 ,探讨大肠癌患者外周血中CK2 0的表达与微转移的关系 ,研究大肠癌与p5 3基因突变的关系。方法 运用荧光监测技术结合RT -PCR ,检测 3 6例大肠癌患者术前和术后不同时间外周血中的CK2 0的表达。用SSCP技术对大肠癌患者进行p5 3基因的突变检测。结果 3 6例患者的外周血中检出CK2 0表达的有 2 7例 ,阳性率为 75 % ,手术后 2 4h患者外周血中检出CK2 0表达的有 3 0例 ,阳性率为 83 3 % ,术后早期化疗结束后仅 13例为阳性 ,阳性率为 3 6%。 3 6例患者的p5 3基因的SSCP突变检测中未检测到突变。结论 运用荧光PCR技术检测外周血中CK2 0的表达可以监测癌症微转移的发生 ,简便快速 ,检出率高。
Objective To establish an efficient and rapid detection method of cytokeratin 20 (CK20) in peripheral blood to explore the relationship between the expression of CK20 and the micrometastasis in peripheral blood of patients with colorectal cancer, Relationship between gene mutations. Methods Fluorescence monitoring and RT-PCR were used to detect the expression of CK20 in peripheral blood of 36 patients with colorectal cancer before and after surgery. Detection of p5 3 mutation in colorectal cancer patients by SSCP technique. Results Twenty-seven CK2 0 were detected in peripheral blood of 36 patients with a positive rate of 75%. Thirty (40%) CK2 0 were detected in peripheral blood 24 h after operation, the positive rate was 83 3 %, Only 13 cases were positive after the early postoperative chemotherapy, the positive rate was 36%. No mutation was detected in SSCP mutation of p5 3 gene in 36 patients. Conclusion The detection of CK20 in peripheral blood by fluorescence PCR can monitor the occurrence of micro-metastasis of cancer, which is simple and rapid with high detection rate.
