目的研究组织学炎症对BPH患者下尿路症状(LUTS)的影响。方法选取2001年—2006年间河南省人民医院泌尿外科收治的以BPH为诊断并接受开放性前列腺切除术的患者216例,年龄54~92岁(71.3±6.9岁)。对手术标本切片进行炎症细胞的免疫组化判定,分析炎症细胞分布与相关LUTS症状的关系。结果随着炎症细胞浸润程度加重,LUTS逐渐加重(P<0.05),T淋巴细胞和巨噬细胞浸润程度与IPSS呈正相关(P<0.05),而B细胞与IPSS无明显关系。巨噬细胞浸润程度与最大尿流率(Qmax)呈负相关(P<0.01),而T细胞浸润程度与不稳定膀胱发生率呈正相关(P<0.01)。结论 BPH组织学炎症能加重LUTS,其中,T细胞和巨噬细胞浸润明显增加了IPSS评分,T细胞浸润明显增加不稳定性膀胱的发生,而巨噬细胞浸润能明显导致Qmax下降,是BPH进展的重要原因。
Objective To investigate the effect of histological inflammation on lower urinary tract symptoms (LUTS) in patients with BPH. Methods A total of 216 patients with BPH who underwent open prostatectomy who were admitted to Department of Urology, Henan Provincial People ’s Hospital from 2001 to 2006 were selected, ranging in age from 54 to 92 years (71.3 ± 6.9 years). The specimens of the surgical specimens were subjected to immunohistochemical determination of inflammatory cells to analyze the relationship between inflammatory cell distribution and the related LUTS symptoms. Results As the infiltration of inflammatory cells increased, LUTS gradually increased (P <0.05). The infiltration of T lymphocytes and macrophages was positively correlated with IPSS (P <0.05), while B cells had no significant relationship with IPSS. The degree of infiltration of macrophages was negatively correlated with Qmax (P <0.01), while the infiltration of T cells was positively correlated with the incidence of unstable bladder (P <0.01). Conclusion Histological inflammation of BPH can aggravate LUTS. Among them, infiltration of T cells and macrophages significantly increased IPSS score, T cell infiltration significantly increased the occurrence of unstable bladder, while macrophage infiltration could significantly reduce Qmax, which was the progression of BPH The important reason.
