目的:对比研究不同病因间股骨头缺血性坏死的CT表现。材料和方法:收集经临床影像学检查证实的60个股骨头缺血性坏死的髋关节,根据病因将其分为特发性、酒精性、激素性、股骨颈骨折性四类,将这四类股骨头缺血坏死的CT分期表现进行对比分析。结果:不同病因间股骨头缺血性坏死的CT各个分期表现相同。均表现为:Ⅰ期股骨头“星征”改变,斑片状硬化并骨质疏松;Ⅱ期“星征”消失,骨质斑片状硬化和或囊状透光区;Ⅲ期骨质硬化、囊变,“新月”征阳性,骨质轻度碎裂,关节面轻度塌陷;Ⅳ、Ⅴ期股骨头变形,骨质碎裂,关节面塌陷,Ⅴ期在Ⅳ期基础上合并髋关节退行性改变。结论:对比不同病因间股骨头缺血性坏死的分期CT表现是相同的。
Objective: To compare the CT findings of avascular necrosis of the femoral head between different etiologies. MATERIALS AND METHODS: Sixty femoral head avascular necrosis hip joints confirmed by clinical imaging were collected and divided into four groups: idiopathic, alcoholic, hormonal and femoral neck fractures according to the cause. Comparative analysis of CT staging of avascular necrosis of femoral head. Results: The different stages of avascular necrosis of the CT showed the same staging. All showed as follows: stage Ⅰ femoral head “star sign” change, patchy sclerosis and osteoporosis; Ⅱ “star sign” disappeared, bone patchy sclerosis and saccular light transmission area; Ⅲ Stage osteopetrosis, cystic change, positive crescent sign, mild fragmentation of bone, slight collapse of articular surface, femoral head degeneration, osteochondrosis, articular surface collapse in stages Ⅴ and Ⅴ, Ⅳ on the basis of combined degeneration of the hip joint. Conclusions: The staging CT findings of femoral head avascular necrosis between different etiologies are the same.
