目的探讨过敏性紫癜(HSP)患儿急性期外周血白细胞转录因子FOXP3基因表达情况及其与发病机制间的关系。方法 2008年11月至2009年4月在天津市儿童医院风湿免疫科住院的HSP患儿(HSP组)50例、KD患儿(KD组)34例及门诊择期行扁桃体切除术体检无异常儿童(对照组)50例,检测其外周血白细胞FOXP3 mRNA表达(剔除非特异性扩增标本)。结果 3组共计44例最终进入统计学分析,其中HSP组17例,KD组14例,对照组13例。HSP组急性期外周血白细胞FOXP3 mRNA的表达与对照组比较显著降低,差异有统计学意义(P<0.05);KD组外周血FOXP3 mRNA表达也较对照组减低,差异有统计学意义(P<0.05)。但HSP组与KD组间比较,差异无统计学意义(P>0.05)。结论 FOXP3表达减低可能为导致儿童期系统性血管炎免疫失衡的共同原因,但并非HSP的特异性改变。
Objective To investigate the expression of peripheral blood leukocyte transcription factor FOXP3 in children with Henoch-Schonlein Purpura (HSP) and its relationship with pathogenesis. Methods Fifty patients with HSP (HSP group), 34 children with KD (KD group) and outpatients undergoing elective tonsillectomy were enrolled in the Department of Rheumatology, Tianjin Children’s Hospital from November 2008 to April 2009 without abnormalities (Control group) 50 cases, detection of peripheral blood leukocyte FOXP3 mRNA expression (excluding nonspecific amplification specimens). Results A total of 44 cases in 3 groups finally entered the statistical analysis, including 17 cases in HSP group, 14 cases in KD group and 13 cases in control group. The expression of FOXP3 mRNA in peripheral blood leukocytes in HSP group was significantly lower than that in control group (P <0.05), and the expression of FOXP3 mRNA in peripheral blood of KD group was also lower than that in control group (P < 0.05). However, no significant difference was found between HSP group and KD group (P> 0.05). Conclusions Reduced expression of FOXP3 may be the common cause of immune imbalance in systemic vasculitis in childhood, but not the specific change of HSP.
