目的研究丙型肝炎病毒(HCV)感染所致的肝功能损伤患者中,血清瘦素水平和外周血B细胞、T细胞亚群水平的变化,探究感染在损伤肝功能过程中的影响。方法取本院58例HCV感染患者作为观察组,34例门诊健康体检者作为对照组,采用放射免疫分析法检测血清瘦素水平,单克隆抗体法检测外周血B细胞、T细胞亚群水平。结果和对照组比较,HCV感染患者血清瘦素水平、外周血B细胞数显著升高,差异有统计学意义(P<0.01);CD3、CD4、CD4/CD8则显著降低,差异有统计血意义(P<0.01);HCV感染患者的血清瘦素水平与肝功能指标丙氨酸氨基转移酶(ALT)、血清总胆红素(TBil)水平呈明显的正相关。结论检测HCV感染患者的血清瘦素水平、外周血B细胞和T细胞亚群可以反映其机体免疫功能及肝脏受损情况,有效指导临床上对此类患者的诊疗工作。
Objective To investigate the changes of serum leptin levels and peripheral blood B lymphocytes and T lymphocyte subsets in patients with hepatic impairment induced by hepatitis C virus (HCV) infection and explore the influence of infection on the liver function. Methods Fifty-eight patients with HCV infection in our hospital were selected as observation group and 34 healthy subjects were selected as control group. Serum leptin level was measured by radioimmunoassay. Peripheral blood B cells and T cell subsets were detected by monoclonal antibody. Results Compared with the control group, the serum leptin levels and the number of peripheral blood B cells in HCV-infected patients were significantly increased (P <0.01), while the levels of CD3, CD4 and CD4 / CD8 were significantly decreased (P <0.01). The level of serum leptin in patients with HCV infection was positively correlated with the levels of alanine aminotransferase (ALT) and total bilirubin (TBil). Conclusion Detection of serum leptin level in HCV-infected patients, peripheral blood B-cell and T-cell subsets can reflect the immune function and liver damage, effectively guiding clinical diagnosis and treatment of such patients.
