目的探讨多层螺旋CT血管成像(MSCTA)及其后处理技术在烟雾病诊断中的应用价值。方法 21例经DSA证实的烟雾病病例均行CT平扫及MSCTA检查,将原始数据传输到ADW4.3工作站上进行容积再现(VR)、最大密度投影(MIP)、多平面容积重建(MPR)等多种后处理并结合原始数据进行综合分析。结果 21例患者,平扫显示颅内缺血性改变8例、出血7例,出血合并梗塞3例、脑发育不良2例,大致正常1例。CTA可显示不同程度的颈内动脉和(或)大脑前动脉、大脑中动脉狭窄或闭塞。21例病例均可清晰显示颅底部异常增生的侧支血管网。结论 MSCT血管成像及其后处理可以清晰显示烟雾病的血管改变情况,尤其重建图像可以直观显示病变周围空间关系,可为临床诊断及该病治疗方案的制定提供重要依据。
Objective To investigate the value of multi-slice spiral CT angiography (MSCTA) and its post-processing techniques in the diagnosis of moyamoya disease. Methods Twenty - one cases of moyamoya disease confirmed by DSA were scanned by CT and MSCTA. The raw data were transmitted to ADW4.3 workstation for VR, MPI, MIP, After a variety of post-processing and combined with the original data for comprehensive analysis. Results In 21 patients, 8 cases showed intracranial ischemic changes by plain scan, 7 cases had hemorrhage, 3 cases had hemorrhage complicated with infarction, 2 cases had dysplasia of brain and 1 case was almost normal. CTA can show varying degrees of internal carotid artery and / or anterior cerebral artery, middle cerebral artery stenosis or occlusion. 21 cases of patients can clearly show abnormal basal skull base collateral blood vessels network. Conclusion MSCT angiography and postprocessing can clearly show the vascular changes of moyamoya disease. In particular, reconstructed images can directly show the spatial relationship of the lesions, which can provide important evidences for clinical diagnosis and treatment of the disease.
