目的 探讨新生儿缺氧缺血性脑病(HIE)血清中T_3、T_4、TSH的变化以及二者之间的关系。方法 本文采用放射免疫分析法检测了30例HIE患儿,45例新生儿肺炎患儿及20例正常足月新生儿血清中T_3、T_4、TSH的含量。结果 新生儿HIE组血清T_3、T_4明显低于正常对照组,差异有显著性(P0.05)。HIE组T_3较新生儿肺炎组明显降低差异有显著性(P<0.01)。轻度、中度、重度HIE时,血清T_3改变呈有规律性的梯度降低,差异有显著性(P<0.05)。血清T_4重度HIE与对照组比较明显降低,差异有显著性(P<0.05)。TSH各组间变化不明显,差异无显著性(P>0.05)。结论 动态观察HIE患儿血清T_3、T_4、TSH的变化,对临床判断HIE分度,观察疗效,估计预后有一定的意义。
Objective To investigate the changes of serum T_3, T_4 and TSH in neonates with hypoxic-ischemic encephalopathy (HIE) and the relationship between the two. Methods The levels of T_3, T_4 and TSH in 30 HIE infants, 45 neonates with pneumonia and 20 normal term newborns were detected by radioimmunoassay. Results Serum levels of T_3 and T_4 in neonatal HIE group were significantly lower than those in normal control group (P <0.01, P <0.05). The level of T_3 in neonatal pneumonia group was significantly lower than that in control group (P < O.05), T_4 change was not significant, the difference was not significant (P> 0.05). There was a significant difference between HIE group and neonatal pneumonia group (P <0.01). Mild, moderate, severe HIE, serum T_3 changes showed a regular gradient decreased, the difference was significant (P <0.05). Serum T_4 severe HIE significantly decreased compared with the control group, the difference was significant (P <0.05). There was no significant difference between TSH groups (P> 0.05). Conclusions The dynamic observation of the changes of serum T 3, T 4 and TSH in children with HIE has certain significance in judging the HIE index, observing the curative effect and estimating the prognosis.
