目的:应用逆转录多聚酶链反应(RT-PCR)法检测肺癌外周血中的表达情况,并探讨其表达的临床意义。方法:用RT-PCR法检测98例肺癌患者外周血CK19 mRNA表达。结果:NSCLC组患者外周血CK19 mRNA的阳性率为77.55%(76/98),显著高于良性病变组的3.57%(1/28)和健康对照组的0,χ2=73.68,P<0.05;Ⅱ期〔87.50%(35/40)〕和Ⅲ期〔88.89%(24/27)〕NSCLC患者的阳性率显著高于Ⅰ期〔54.84%(17/31)〕患者的阳性率,χ2=13.453,P<0.05;淋巴结转移阳性和阴性患者的外周血中CK19 mRNA阳性率分别为89.55%(60/67)和51.61%(16/31),差异有统计学意义,χ2=17.523,P<0.05;而不同病理类型NSCLC患者外周血CK19mRNA阳性率之间差异无统计学意义,χ2=0.414,P>0.05。结论:肺癌患者外周血中CK19 mRNA表达较高可能与肺癌转移有关,检测CK19 mRNA对于早期判断患者转移和复发有一定的临床意义。
Objective: To detect the expression of lung cancer cells by reverse transcription polymerase chain reaction (RT-PCR) and to investigate its clinical significance. Methods: The expression of CK19 mRNA in peripheral blood of 98 patients with lung cancer was detected by RT-PCR. Results: The positive rate of CK19 mRNA in peripheral blood of patients with NSCLC was 77.55% (76/98), which was significantly higher than that of benign lesion (3.57%, 1/28) and that of healthy controls (χ2 = 73.68, P <0.05). The positive rate of patients with NSCLC in stage Ⅱ (87.50% (35/40)) and in stage Ⅲ (88.89% (24/27)) was significantly higher than that in stage Ⅰ (54.84%, 17/31), χ2 = 13.453 (P <0.05). The positive rates of CK19 mRNA in peripheral blood of positive and negative patients with lymph node metastasis were 89.55% (60/67) and 51.61% (16/31) respectively, the difference was statistically significant, χ2 = 17.523, P <0.05 The positive rates of CK19 mRNA in peripheral blood of NSCLC patients with different pathological types had no significant difference (χ2 = 0.414, P> 0.05). Conclusion: The high expression of CK19 mRNA in the peripheral blood of patients with lung cancer may be related to the metastasis of lung cancer. Detecting CK19 mRNA may be of clinical significance in early diagnosis of patients with metastasis and recurrence.
