目的 探究巨大子宫肌瘤采用腹腔镜辅助阴式全子宫切除术的治疗效果?方法 于2018年3月至2019年4月这一期间,选取本院收治的巨大子宫肌瘤患者96例为研究对象,根据数字表法的分组规则,将以上患者分为两组,即对比组?试验组,每组人数48例?对比组予以传统开腹手术治疗,试验组予以腹腔镜辅助阴式全子宫切除术治疗?分析两组患者的手术情况,统计并对比手术时间?手术出血量?肛门排气时间?下地活动时间?留置导尿时间及住院天数;分析并比较两组患者的雌激素?孕激素?表皮生长因子受体阳性细胞百分比;研究两组患者的并发症发生情“,”Objective To investigate the therapeutic effect of laparoscopic-assisted total vaginal hysterectomy for giant uterine fibroids. Methods From March 2018 to April 2019, 96 patients with giant uterine fibroids admitted to our hospital were selected as the research objects. According to the grouping rules of number table method,the patients were divided into two groups, namely comparison and experimental group, with 48 patients in each group. The contrast group was treated with traditional laparotomy, and the experimental group was treated with laparoscopic-assisted total vaginal hysterectomy. The operative conditions of the two groups were analyzed, and the operative time, operative blood loss, anal exhaust time, underground movement time, indwelling catheter time and hospitalization days were statistically compared. The percentage of estrogen, progesterone and epidermal growth factor receptor positive cells in two groups were analyzed and compared. The complications such as abdominal organ injury, incision infection and subcutaneous emphysema were studied. Results The operation time of experimental group was longer than that of contrast group (P<0.05). The amount of blood loss in the experimental group was less than that in the control group, and the time of anal exhaust, ground movement, indwelling catheter and hospital stay were shorter than that in the control group, with statistical significance (P<0.05). After treatment, the percentage of estrogen, progesterone and epidermal growth factor receptor positive cells in two groups were less than before treatment (P<0.05); before treatment, there was no significant difference in the percentage of estrogen, progesterone and epidermal growth factor receptor positive cells between the two groups (P<0.05) the percentages of estrogen, progesterone and epidermal growth factor receptor positive cells in experimental group after treatment were (1.26±0.36)%, (1.46±0.25)%, (1.17±0.35)% respectively. After treatment, the percentage of estrogen, progesterone and epidermal growth factor receptor positive cells in the control group was (1.48±0.47)%, (1.63±0.37)% and (1.68±0.53)%, respectively. There was no abdominal organ injury in the experimental group, and 1 case of incision infection and subcutaneous emphysema. The total incidence of complications was 4.17% (2/48), which was significantly lower than 16.67% (8/48) in the control group, and the difference between groups was statistically significant (P<0.05). Conclusion Laparoscopic-assisted total vaginal hysterectomy for giant uterine fibroids has significant therapeutic effect and high safety, and can effectively reduce the hormone level of patients.
