目的对患偏头痛的41例空勤人员做脑地形图(BEAM)检查的同时又做经颅多普勒(TCD)检查,以便作出诊断和评估。方法采用经颅多普勒血流诊断仪和脑电地形图仪进行检测。结果BEAM检测结果正常的4例(10%),异常37例(90%);TCD检测结果正常的5例(12%),异常的36例(88%)。BEAM与TCD表现有相关性:BEAM表现α波功率值在局部不对称,TCD表现相应部位血管血流速度的异常;BEAM表现前头部α波功率值高于后头部,TCD表现后头部脑血流速度异常。BEAM表现全导联α波功率值降低,TCD表现多根血管血流速度降低;BEAM局限性异常的,TCD表现单根或数量很少的血管血流速度异常;BEAM表现阵发性异常趋势越明显,TCD检查血流速度越不稳定。结论BEAM异常率与TCD异常率很接近,两者之间有密切的相关性。对空勤人员的偏头痛患者做BEAM和TCD检查,可以利用它们之间的相对特异性和敏感性,还可以弥补它们的不足和疏漏,做出较为完善的诊断与评估。
Objective To conduct a transcranial Doppler (TCD) examination of 41 BEAM patients with migraine headache for diagnosis and evaluation. Methods Transcranial Doppler flow cytometer and EEG topograph were used for detection. Results Four patients (10%) with abnormal BEAM results and 37 patients (90%) had abnormal BEAM results. Five patients (12%) had normal TCD test results and 36 patients (88%) had abnormal TCD test results. BEAM and TCD showed a correlation: BEAM performance α wave power value in the local asymmetry, TCD performance of the corresponding parts of the vascular blood flow velocity abnormalities; BEAM frontal α wave power value higher than the head, TCD performance after the head Abnormal cerebral blood flow velocity. BEAM showed a decrease in all-lead α-wave power and a decrease in blood vessel velocity in TCD. Abnormal BEAM showed a single or small number of abnormal blood flow velocity in the BEAM. Obviously, TCD examination of blood flow instability. Conclusions The abnormal rate of BEAM is very close to the TCD abnormal rate, and there is a close correlation between the two. Performing BEAM and TCD examinations for migrant patients with aircrew can make use of the relative specificity and sensitivity between them and make up for their deficiencies and omissions to make a sound diagnosis and assessment.
