目的:研究白细胞滤除对不同时间制备的新鲜冰冻血浆(FFP)凝血因子的影响。方法:随机选取无偿献血400ml全血标本60份,每份用无菌接驳机分为4份,其中2份去除白细胞为去白组,另2份不去除白细胞为未去白组,在8h内、8~18h制备FFP,即为8小时冰冻血浆(FP8)、去白8小时冰冻血浆(去白FP8)和FP18、去白FP18,4种血浆分别检测纤维蛋白原(Fib)、8因子(FⅧ)及5因子(FⅤ)含量,并比较合格率。结果:FP8与去白FP8相比,2组的Fib、FⅧ和FⅤ因子含量差异均无统计学意义(均P>0.05);FP18与去白FP18相比,FⅧ、FⅤ因子含量差异有统计学意义(P<0.05),Fib含量差异无统计学意义(P>0.05)。8h内制备的FFP与8~18h制备的FFP相比,不论是白细胞过滤前、后,前者的Fib、FⅧ和FⅤ因子含量均显著高于后者(P<0.05)。FP8、去白FP8中FⅧ含量合格率分别为90.00%、86.67%,差异无统计学意义(χ2=0.32,P>0.05);FP18、去白FP18中FⅧ合格率分别为76.67%、56.67%,差异有统计学意义(χ2=5.40,P<0.05)。结论:8~18h制备的FFP也能达到现行质量标准,符合临床要求,而去白细胞FFP最好在8h内制备。
Objective: To study the effect of leukocyte filtration on the coagulation factors of fresh frozen plasma (FFP) prepared at different times. Methods: A total of 60 samples of 400 ml whole blood were collected randomly, each of which was divided into 4 parts by aseptic feeder, 2 of which were leukocyte removal, and the other 2 were of non-white matter removal without leukocytes. After 8 h (FP8), 8 hours of de-to-white frozen plasma (FP8) and FP18, 4 days of FP18, and 4 kinds of plasma were used to detect fibrinogen (Fib), 8-factor (F Ⅷ) and 5 factors (F Ⅴ) content, and compare the pass rate. Results: There was no significant difference in the levels of Fib, FⅧ and FV between FP8 and FP8 (all P> 0.05). The differences of FⅧ and FV between FP18 and FP18 were statistically significant Significance (P <0.05), Fib content was no significant difference (P> 0.05). The FFP, FⅧ and FV contents of FFP prepared in 8h were significantly higher than that of FFP prepared in 8 ~ 18h (P <0.05), both before and after leukocyte filtration. FP8 and FP8 were 90.00% and 86.67%, respectively (χ2 = 0.32, P> 0.05). The pass rates of FⅧ in FP18 and FP18 were 76.67% and 56.67%, respectively, The difference was statistically significant (χ2 = 5.40, P <0.05). Conclusion: The FFP prepared from 8 to 18 hours can meet the current quality standards and meets the clinical requirements. However, FFP from 8 to 18 hours is best prepared within 8 hours.
