Objective To investigate the relationship between serum C-peptide and lower-extremity arterial disease (LEAD) in type 2 diabetes mellitus (T2DM) and related risk factors. Methods According to the pathological changes of lower extremity arteries in T2DM patients, the lesion group (LEAD group, 121 cases) and the simple group (T2DM group, 90 cases) were divided into two groups. The body mass index (BMI), fasting C-peptide, glycemic 2 h C peptide, glycosylated hemoglobin, fasting blood glucose and fasting insulin were detected in the two groups. Results The fasting C-peptide and 2h C-peptide in the LAD group were lower than those in the T2DM group, while the HbA1c was higher in the LEAD group than in the T2DM group (P <0.05). Conclusions Serum C-peptide, especially glucose-loaded 2-h C-peptide, is the protective factor of LEAD in T2DM patients, and its decline indicates the development of LEAD in T2DM patients.
