目的用Cox回归法研究E-cadherin在结直肠癌中的表达及与患者预后的相关性。方法以本院收治的结直肠癌患者68例为研究对象,用免疫组织化学SP法检测癌组织中E-cadherin的表达水平;同时应用Cox回归法对影响患者预后的相关因素进行分析,探讨E-cadherin与患者生存期的关系。结果 E-cadherin在肠癌组织中的阳性率为39.7%(27/68例),缺失表达率为60.3%(41/68例)。Cox回归分析显示,结直肠癌预后与患者TNM分期(OR=4.11,95%CI:1.74~8.69,P<0.05)、淋巴结转移(OR=3.66,95%CI:1.23~5.89,P<0.05)及E-cadherin阳性表达(OR=1.98,95%CI:1.41~6.45,P<0.05)有关;3年内无疾病进展生存率,E-cadherin阳性组为58.1%,阴性组为46.6%,阳性组显著高于阴性组,差异有统计学意义(P<0.05)。结论结直肠癌组织中E-cadherin阳性表达者预后较好,E-cadherin可作为结直肠癌预后独立相关因素。
Objective To study the expression of E-cadherin in colorectal cancer and its relationship with the prognosis of patients by Cox regression. Methods Sixty-eight patients with colorectal cancer admitted to our hospital were enrolled in this study. The expression of E-cadherin in cancer tissues was detected by immunohistochemical SP method. Meanwhile, Cox regression analysis was used to analyze the influencing factors of prognosis in patients with E The relationship between -cadherin and patient survival. Results The positive rate of E-cadherin in colorectal cancer was 39.7% (27/68 cases) and the rate of deletion was 60.3% (41/68 cases). Cox regression analysis showed that the prognosis of colorectal cancer was significantly correlated with TNM stage (OR = 4.11, 95% CI: 1.74-8.69, P <0.05) and lymph node metastasis (OR = 3.66, 95% CI: 1.23-5.80, P < (OR = 1.98, 95% CI: 1.41-6.45, P <0.05). The disease-free survival rate in 3 years was 58.1% in E-cadherin positive group and 46.6% in negative group Significantly higher than the negative group, the difference was statistically significant (P <0.05). Conclusions The positive expression of E-cadherin in colorectal cancer tissues is better than that of E-cadherin. E-cadherin may be an independent prognostic factor for colorectal cancer.
