目的 :讨论凝血酶在前置胎盘剥宫产术中产后出血的治疗中能够起到的作用。方法 :回顾性分析我院2013年10月-2014年10月期间在我院施术的前置胎盘剖宫产术中患者46例,其中把运用凝血酶治疗前置胎盘剖宫产术分为实验组23例,把没有运用的患者分为对照组共23例,试验组中手术采用肾上腺素稀释液喷洒出血面后,在患者出血的部位快速的敷上放置了凝血酶的明胶海棉,随后加上热盐水垫压迫止血;对照组采用传统的宫腔填塞纱布条止血。手术后,分别对实验组和对照组进行出血量、出血时间、术后病率和止血效果的检查,得出实验结果。结果 :运用凝血酶治疗前置胎盘剖宫产术的小组出血时间为(28.5±8.12)S,平均出血量为(76±9.65)ml,止血效果100.0%,术后病率为31.5%。未运用凝血酶治疗前置胎盘剥宫产术中产后出血的小组,出血时间为(75.61±13.85)S,平均出血量为(165±39)ml,止血效果77.7%,术后病率28.5%。两组的出血时间、出血量、止血效果、术后病率均有统计学意义(P<0.05)。结论 :在前置胎盘剖宫产术中胎盘剥离面大出血的手术中,凝血酶具有良好的止血效果和治愈效果,是值得临床使用的药物,可以进行推广。
Objective: To discuss the role of thrombin in the treatment of postpartum hemorrhage in placenta previa. Methods: A retrospective analysis of our hospital from October 2013 -2014 in October period in our hospital in 46 cases of placenta previa patients with placenta previa, of which the use of thrombin in the treatment of placenta previa were divided into In the experimental group, 23 patients were divided into control group and 23 patients. In the experimental group, epinephrine diluted solution was sprayed on the bleeding surface, and thrombin gelatin sponge was deposited on the site of bleeding. Then add hot saline pad compression hemostasis; control group using the traditional uterine packing gauze to stop bleeding. After the operation, the experimental group and the control group were respectively examined for the amount of bleeding, the bleeding time, the postoperative morbidity and the hemostatic effect, and the experimental results were obtained. Results: The bleeding time of the group treated with thrombin in the placenta previa was (28.5 ± 8.12) s, the average amount of bleeding was (76 ± 9.65) ml, the hemostatic effect was 100.0% and the postoperative morbidity was 31.5%. The bleeding time was 75.61 ± 13.85 s, the mean bleeding volume was (165 ± 39) ml, the hemostatic effect was 77.7%, the postoperative morbidity was 28.5% . The bleeding time, bleeding volume, hemostatic effect and postoperative morbidity of the two groups were all statistically significant (P <0.05). Conclusion: In the placenta previa placenta previa placental hemorrhage surgery, thrombin has a good hemostatic effect and healing effect, is worthy of clinical use of drugs, can be promoted.
