目的探讨Notch1受体和表皮生长因子受体(EGFR)在舌鳞癌中的表达,并分析Notch1和EGFR潜在的关系,为舌鳞癌的临床治疗提供支持。方法选择2010年1月—2014年12月间杭州师范大学附属医院收集的71例舌鳞癌标本、62例舌黏膜白斑标本和17例正常舌黏膜标本作为研究对象。采用免疫组化法测定3组标本中Notch1和EGFR的表达,并采用SPSS 16.0统计软件对3组数据结果进行统计学分析。结果正常舌黏膜、舌黏膜白斑标本中Notch1的阳性表达主要集中在角化层,基底层无表达;而EGFR的阳性表达主要集中在基底层,角化层和颗粒层则无表达。舌鳞癌标本中,高分化程度标本中Notch1的阳性细胞率越高(P<0.05),而TNM分期越高、有舌肌层浸润、有淋巴转移标本的Notch1阳性细胞率越低(P<0.05);相反,高分化程度标本中EGFR的阳性细胞率越低(P<0.05),TNM分期越高、有舌肌层浸润、有淋巴转移标本的EGFR阳性细胞率越高(P<0.05)。结论舌黏膜上皮中Notch1受体的表达可导致细胞的分化,Notch1水平的下调可抑制舌鳞癌,EGFR的表达在一定程度上抑制Notch1的表达,两者在舌鳞癌的发展中可能存在交互作用。
Objective To investigate the expression of Notch1 receptor and epidermal growth factor receptor (EGFR) in tongue squamous cell carcinoma and to analyze the potential relationship between Notch1 and EGFR and to provide support for the clinical treatment of tongue squamous cell carcinoma. Methods Totally 71 specimens of tongue squamous cell carcinoma, 62 specimens of lingual mucosa and 17 specimens of normal lingual mucosa collected from the Affiliated Hospital of Hangzhou Normal University from January 2010 to December 2014 were selected as the study objects. The expressions of Notch1 and EGFR in three groups of specimens were detected by immunohistochemistry. The data of three groups were statistically analyzed by using SPSS 16.0 statistical software. Results The positive expression of Notch1 in the tongue mucosa and lingual mucosa leukoplakia mainly focused on the keratinized layer but not in the basal layer. However, the positive expression of EGFR was mainly located in the basal layer but not in the keratinized layer and granular layer. In tongue squamous cell carcinoma specimens, the percentage of Notch1 positive cells was higher in well differentiated specimens (P <0.05), while the higher TNM staging was found in tongue myenteric layer. The rate of Notch1 positive cells in lymphoid specimens was lower (P < 0.05). In contrast, the positive rate of EGFR positive cells was lower in highly differentiated specimens (P <0.05). The higher the TNM staging was, . Conclusion The expression of Notch1 receptor in lingual mucosa epithelium may lead to cell differentiation. Downregulation of Notch1 may inhibit tongue squamous cell carcinoma. The expression of EGFR may inhibit the expression of Notch1 to a certain extent. Both may play an important role in the development of tongue squamous cell carcinoma effect.
