目的探讨基质金属蛋白酶-1(matrix metalloproteinase-1,MMP-1)及组织基质金属蛋白酶抑制物(tissue inhibitor of metalloproteinase,TIMP-1)表达在盆腔器官脱垂中的意义。方法收集因盆腔器官脱垂行盆底重建手术的患者60例为脱垂组,同期因妇科良性肿瘤行子宫全切术且既往无盆腔器官脱垂或手术史的患者35例为非脱垂组,采用实时定量PCR法检测MMP-1 m RNA及TIMP-1 m RNA在两组阴道前壁中表达。结果脱垂组患者阴道前壁组织中的MMP-1 m RNA表达量(0.03±0.01)显著高于非脱垂组(0.02±0.00),差异有统计学意义(P<0.05)。非脱垂组TIMP-1 m RNA表达量(0.54±0.07)高于脱垂组(0.42±0.06),差异有统计学意义(P<0.05)。结论阴道前壁MMP-1m RNA表达增加与MMP-1/TIMP-1比例失调可能是导致盆腔器官脱垂的原因之一。
Objective To investigate the significance of matrix metalloproteinase-1 (MMP-1) and tissue inhibitor of metalloproteinase (TIMP-1) in pelvic organ prolapse. Methods Totally 60 patients with pelvic floor reconstruction due to pelvic organ prolapse were treated as prolapse group. 35 patients with history of hysterectomy and previous pelvic organ prolapse or surgery were non-prolapsed Real-time quantitative PCR was used to detect the expression of MMP-1 mRNA and TIMP-1 mRNA in the anterior vaginal wall. Results The expression of MMP-1 m RNA in the anterior vaginal tissue of the prolapsed group was significantly higher than that of the non-prolapsed group (0.03 ± 0.01), the difference was statistically significant (P <0.05). The expression of TIMP-1 m RNA in non-prolapsed group (0.54 ± 0.07) was higher than that in prolapsed group (0.42 ± 0.06), the difference was statistically significant (P <0.05). Conclusions The imbalance between MMP-1mRNA and MMP-1 / TIMP-1 in anterior vaginal wall may be one of the reasons leading to pelvic organ prolapse.
