目的探讨异氟醚后处理对大鼠肾脏缺血再灌注损伤(ischemia-reperfusion injury,IRI)的影响。方法健康雄性Wistar大鼠36只,体质量300~350 g,采用随机数字表法分为3组(n=12):假手术(S)组、肾脏缺血再灌注(IR)组和异氟醚后处理(ISO)组。采用夹闭双侧肾蒂60 min、恢复灌注4 h,建立大鼠肾脏缺血再灌注模型。ISO组于再灌注前开始吸入2.5%异氟醚持续5 min。再灌注4 h时处死大鼠,取肾组织,分别采用黄嘌呤氧化酶法、硫代巴比妥法测定超氧化物歧化酶(superoxide dismutase,SOD)活性、丙二醛(malondialdehyde,MDA)含量,光镜下观察病理学结果,采用免疫组化法测定肾组织CD44和肿瘤坏死因子(tumor necrosis factor,TNF)的表达水平。结果免疫组化结果示:与S组比较,IR组肾组织MDA含量升高,SOD活性降低,CD44和TNF-α的表达上调(P<0.05);与S组比较,ISO组MDA含量升高、SOD活性降低、肾组织CD44和TNF-α的表达无显著差异(P>0.05);与I/R组比较,ISO组MDA含量降低、SOD活性升高、肾组织CD44和TNF-α的表达显著下调(P<0.05)。光镜结果示,S组大鼠组织结构基本正常,ISO组大鼠肾组织病理损伤明显轻于IR组。结论异氟醚后处理可以减轻大鼠肾脏IRI。
Objective To investigate the effect of isoflurane postconditioning on renal ischemia-reperfusion injury (IRI) in rats. Methods Thirty-six healthy male Wistar rats weighing 300-350 g were randomly divided into 3 groups (n = 12): sham group (S), IR group and isoflurane Ether after-treatment (ISO) group. The bilateral renal pedicle was implanted for 60 min and the perfusion was resumed for 4 h. Rat renal ischemia-reperfusion model was established. The ISO group started inhalation of 2.5% isoflurane for 5 min before reperfusion. The rats were sacrificed at 4 h after reperfusion, and the kidneys were sacrificed. The activities of superoxide dismutase (SOD) and malondialdehyde (MDA) were determined by xanthine oxidase and thiobarbiturine respectively. The pathological results were observed under light microscope. The expression of CD44 and tumor necrosis factor (TNF) in renal tissue were detected by immunohistochemistry. Results The results of immunohistochemistry showed that compared with group S, the content of MDA in renal tissue was increased, the activity of SOD was decreased and the expressions of CD44 and TNF-α were up-regulated in IR group (P <0.05) (P> 0.05). Compared with the I / R group, the content of MDA and the activity of SOD in the ISO group were significantly decreased while the expressions of CD44 and TNF-α in the kidney tissue were increased Significantly decreased (P <0.05). Light microscopy results showed that the tissue structure of rats in S group was basically normal, and the pathological damage of kidney in ISO group was lighter than that in IR group. Conclusion Isoflurane postconditioning can reduce renal IRI in rats.
